Overview

This drug is a combination chemotherapy regimen consisting of dactinomycin (also known as actinomycin D), etoposide, and methotrexate. It is primarily used in the treatment of high-risk gestational trophoblastic neoplasia (GTN) and as salvage therapy for refractory germ cell tumors. The regimen works by combining three cytotoxic agents with distinct mechanisms:

  • Dactinomycin intercalates into DNA and inhibits RNA synthesis.
  • Etoposide inhibits topoisomerase II, leading to DNA strand breaks.
  • Methotrexate is an antimetabolite that inhibits dihydrofolate reductase, blocking DNA synthesis.

The combination exploits synergistic effects to maximize tumor cell kill while managing toxicity profiles. This regimen has demonstrated high complete response rates in GTN and durable remissions in some patients with refractory germ cell malignancies.
