Dalargin is a synthetic hexapeptide and a stable analog of the endogenous opioid peptide leucine-enkephalin. It functions primarily as an agonist at peripheral opioid receptors, particularly the delta and mu subtypes. Developed and extensively utilized in Russia, Dalargin is recognized for its cytoprotective, anti-inflammatory, and antioxidant properties. In clinical settings, it has been traditionally used for the treatment of peptic ulcers and pancreatitis. Currently, it is being investigated in Phase 3 clinical trials by Botkin Hospital for its potential in pharmacological preconditioning to prevent postoperative complications, such as multiple organ dysfunction syndrome (MODS), acute kidney injury (AKI), and myocardial injury after non-cardiac surgery (MINS), in patients undergoing high-risk abdominal surgery. The drug is thought to mitigate ischemia-reperfusion injury and systemic inflammatory responses by modulating key signaling pathways, including the NRF2 pathway.
