Dalbavancin is a second-generation, long-acting lipoglycopeptide antibiotic used for the treatment of acute bacterial skin and skin structure infections (ABSSSI) in adults caused by susceptible Gram-positive organisms, including methicillin-resistant Staphylococcus aureus (MRSA). It was designed as an improved alternative to older glycopeptides like vancomycin and teicoplanin, with modifications that allow for increased potency and once-weekly dosing. Dalbavancin exerts its bactericidal effect primarily by inhibiting bacterial cell wall biosynthesis. It binds to the terminal D-alanyl-D-alanine residues of peptidoglycan precursors, preventing their incorporation into the cell wall matrix. This action disrupts cell wall synthesis and leads to bacterial death. Additionally, dalbavancin may alter membrane permeability and RNA synthesis in bacteria. The drug has a prolonged half-life (~14 days), high protein binding (~93%), and achieves good tissue penetration[2][5][6].
