Damasonium alisma extract is an aqueous preparation derived from the shoot systems of the aquatic plant *Damasonium alisma*, commonly known as Starfruit, belonging to the Alismataceae family. The extract is characterized by a diverse phytochemical profile including phenols, flavonoids, alkaloids, tannins, saponins, and steroids. Preclinical research has demonstrated that the extract possesses broad-spectrum biological activities, including antioxidant, antiparasitic, antibacterial, and antineoplastic properties. Its anticancer mechanism of action involves the induction of the intrinsic apoptotic pathway, evidenced by increased mitochondrial membrane permeability, the release of cytochrome c into the cytosol, and subsequent DNA damage and nuclear intensity changes. As an antibacterial agent, it has shown efficacy against various pathogenic strains by disrupting plasma membrane permeability and causing bacterial DNA destruction. While it exhibited lower potency compared to other aquatic plants in certain assays, it remains a significant candidate for natural product-based drug discovery for infectious diseases and oncology.
