Danicamtiv is a novel small molecule cardiac myosin activator under clinical investigation for the treatment of heart failure, particularly primary dilated cardiomyopathy (DCM) and heart failure with reduced ejection fraction (HFrEF)[1][3][4]. It works by directly binding to cardiac myosin, increasing the number of myosins in the force-producing "ON" state and enhancing contractility without affecting intracellular calcium levels[1][5]. Danicamtiv increases systolic force production and left ventricular ejection fraction by slowing cross-bridge turnover through inhibition of ADP release from myosin, which prolongs contraction but may also slow relaxation at higher concentrations[4][5][6]. The drug is being developed to address hypocontractile phenotypes in genetic forms of DCM, especially those related to MYH7 and TTN gene mutations[3].
