Overview

Danicamtiv + itraconazole is a non-standard combination of two pharmaceuticals: danicamtiv, a small molecule cardiac myosin activator under investigation for heart failure with reduced ejection fraction (HFrEF), and itraconazole, an oral antifungal azole class medication used for the treatment of systemic and superficial fungal infections. There is no evidence that this combination is marketed or clinically validated as a unitary therapy. Danicamtiv acts as a direct activator of cardiac myosin, aiming to enhance cardiac contractility by increasing the duration and magnitude of the cardiac muscle's systolic contraction. Itraconazole works as a potent inhibitor of fungal ergosterol synthesis by inhibiting the enzyme lanosterol 14α-demethylase (a cytochrome P450 isoenzyme), interfering with fungal cell membrane function. No data support therapeutic synergy or a unique indication for this combination; drug interaction is plausible due to itraconazole’s strong CYP3A4 inhibition and danicamtiv’s likely hepatic metabolism.
