Overview

Dantrolene is a skeletal muscle relaxant that acts as a post-synaptic muscle relaxant by inhibiting the release of calcium ions from the sarcoplasmic reticulum in skeletal muscle cells. It binds to the ryanodine receptor 1 (RyR1), thereby reducing intracellular calcium and decreasing muscle contraction strength[1][6]. Dantrolene is primarily used for the treatment and prevention of malignant hyperthermia, a rare but life-threatening reaction to certain anesthetics, and is considered the only specific antidote for this condition[3][8]. It is also indicated for spasticity associated with conditions such as spinal cord injury, stroke, multiple sclerosis, or cerebral palsy[2]. Additional uses include management of neuroleptic malignant syndrome and certain cases of severe muscle spasm. The drug does not affect neuromuscular transmission or membrane excitability. Its main adverse effects include drowsiness, dizziness, weakness, fatigue, diarrhea, hepatotoxicity (potentially severe), and others[2][3].
