Dapagliflozin + ACEI refers to the combination therapy of dapagliflozin, a sodium-glucose cotransporter 2 (SGLT2) inhibitor, and an angiotensin-converting enzyme inhibitor (ACEI), such as lisinopril, ramipril, or enalapril. Dapagliflozin lowers blood glucose by inhibiting SGLT2 in the proximal renal tubules, reducing glucose reabsorption and increasing urinary glucose excretion. ACE inhibitors block the conversion of angiotensin I to angiotensin II, leading to vasodilation and reduced aldosterone secretion. This combination is used primarily for patients with type 2 diabetes mellitus (T2DM), chronic kidney disease (CKD), or heart failure to improve glycemic control and reduce cardiorenal events. Clinical studies have shown that this combination can rescue glomerular filtration rate decline and reduce glomerular damage in diabetic kidney disease models[1][3][4]. The combined use is more effective than monotherapy with either agent in preventing cardiovascular events, heart failure hospitalizations, and progression of renal disease[3][4].
