The combination of dapagliflozin, furosemide, and hypertonic saline solution is a therapeutic regimen being investigated by the University of Palermo in the DAPA-TONIC clinical trial. This approach targets acute decompensated heart failure (ADHF) in patients with type 2 diabetes mellitus. Dapagliflozin, an SGLT2 inhibitor, reduces sodium and glucose reabsorption in the proximal tubule, while furosemide inhibits the NKCC2 transporter in the loop of Henle. The addition of small-volume hypertonic saline solution (HSS) acts as an osmotic agent, shifting fluid into the intravascular space to improve renal perfusion and sodium delivery to the nephron. This triple combination aims to maximize fluid removal (diuresis) and sodium excretion (natriuresis), potentially leading to faster weight reduction and shorter hospital stays without adversely affecting renal function.
