SGLT2i + metformin + cofrogliptin is a triple combination antidiabetic regimen consisting of a sodium-glucose cotransporter 2 inhibitor (SGLT2i), metformin, and the novel DPP-4 inhibitor cofrogliptin (HSK7653). In the context of clinical trial NCT07019012, the SGLT2i component is specifically dapagliflozin. Cofrogliptin is a small-molecule dipeptidyl peptidase-4 (DPP-4) inhibitor developed by Haisco Pharmaceutical Group, distinguished by its unique bi-weekly (once every two weeks) dosing schedule, which offers a more convenient alternative to the daily dosing required for other approved DPP-4 inhibitors. The regimen is designed for patients with type 2 diabetes mellitus who have inadequate glycemic control on dual therapy with metformin and an SGLT2 inhibitor. The combination utilizes three complementary mechanisms: SGLT2 inhibition to promote urinary glucose excretion, metformin to reduce hepatic glucose production and improve insulin sensitivity, and cofrogliptin to enhance incretin-mediated insulin secretion.
