Overview

Dapagliflozin + volagidemab is a combination therapy that pairs a sodium-glucose cotransporter 2 (SGLT2) inhibitor with a glucagon receptor antagonist (GRA). Dapagliflozin works by inhibiting SGLT2 in the proximal renal tubules, reducing glucose reabsorption and promoting urinary glucose excretion. Volagidemab is a monoclonal antibody that competes with glucagon for binding at the active site of the glucagon receptor, blocking glucagon action. When used together in type 1 diabetes, this combination improves glycemic control, reduces insulin dosing requirements, and mitigates the risk of diabetic ketoacidosis associated with SGLT2 inhibitor use alone. Clinical trials have shown that the combination therapy achieves better glucose control with increased time in target range (70-180 mg/dL) compared to either insulin alone or SGLT2 inhibitor monotherapy.
