Dapansutrile (OLT1177) is a first-in-class, orally active, selective small-molecule inhibitor of the NLRP3 (nucleotide-binding oligomerization domain, leucine-rich repeat, and pyrin domain-containing 3) inflammasome. Developed by Olatec Therapeutics, it specifically targets the NLRP3 protein to prevent the assembly of the inflammasome complex, which is responsible for the activation of caspase-1 and the subsequent processing of pro-inflammatory cytokines interleukin-1β (IL-1β) and interleukin-18 (IL-18). Unlike other anti-inflammatory treatments, dapansutrile does not interfere with the NLRC4 or AIM2 inflammasomes, nor does it inhibit the production of TNF-α, potentially offering a more targeted safety profile. It is currently under clinical investigation for a variety of inflammatory conditions, including acute gouty arthritis, heart failure, Schnitzler syndrome, and neurodegenerative diseases like Alzheimer's.
