**Dapoxetine + mirodenafil** is a fixed-dose combination of two small molecule drugs, dapoxetine and mirodenafil. Dapoxetine is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of premature ejaculation, while mirodenafil is a phosphodiesterase type 5 (PDE5) inhibitor developed for erectile dysfunction. The combination has been studied for its synergistic effect in patients with lifelong premature ejaculation without erectile dysfunction, demonstrating improved intravaginal ejaculatory latency time (IELT), overall sexual act time (OSAT), and Premature Ejaculation Profile (PEP) scores compared to dapoxetine alone, with a similar safety profile. The two drugs act via separate mechanisms: dapoxetine increases serotonin activity in the synaptic cleft by inhibiting its reuptake, prolonging time to ejaculation, and mirodenafil increases cyclic guanosine monophosphate (cGMP) levels by inhibiting PDE5, resulting in smooth muscle relaxation and enhanced blood flow in the corpus cavernosum. The combination as such does not appear to have an approved brand name or be commercialized but has been evaluated in phase 3 clinical study settings for lifelong premature ejaculation without erectile dysfunction[1][2][3][4].
