Dapsone + metronidazole is a combination of two small molecule antimicrobial agents. Dapsone is a sulfone antibiotic with anti-inflammatory and immunosuppressive properties, primarily used to treat leprosy (Hansen's disease), dermatitis herpetiformis, and acne vulgaris. Its mechanism involves inhibition of dihydropteroate synthase, blocking folic acid synthesis in susceptible organisms, and it also modulates neutrophil activity for its anti-inflammatory effects[2][6]. Metronidazole is a nitroimidazole antimicrobial indicated for the treatment of various anaerobic bacterial and protozoal infections including trichomoniasis, amebiasis, giardiasis, bacterial vaginosis, and certain skin conditions like rosacea. It acts by entering microbial cells where it undergoes reduction to form reactive intermediates that bind DNA and disrupt nucleic acid synthesis in anaerobic bacteria and protozoa[3][5]. The combination may be considered in rare clinical scenarios but carries an increased risk of neurotoxicity due to additive potential for nerve damage[1].
