This is a four-drug combination regimen consisting of daratumumab (a monoclonal antibody targeting CD38), bortezomib (a proteasome inhibitor), cyclophosphamide (an alkylating agent), and dexamethasone (a synthetic glucocorticoid corticosteroid). The regimen is primarily used as induction therapy for newly diagnosed or relapsed/refractory multiple myeloma and systemic light chain (AL) amyloidosis. Daratumumab induces tumor cell death through immune-mediated mechanisms by binding to CD38 on plasma cells. Bortezomib inhibits the 26S proteasome, disrupting protein degradation and leading to apoptosis in malignant cells. Cyclophosphamide crosslinks DNA, inhibiting replication and transcription in rapidly dividing cells. Dexamethasone exerts anti-inflammatory and cytotoxic effects on lymphoid malignancies. This combination has demonstrated high response rates and improved progression-free survival in clinical trials for multiple myeloma and AL amyloidosis[2][6][8].
