Overview

This is a combination regimen consisting of five agents used in the treatment of multiple myeloma. The components are: - **Daratumumab**: A monoclonal antibody targeting CD38, leading to direct tumor cell death and immune-mediated mechanisms. - **Bortezomib**: A proteasome inhibitor that disrupts protein degradation, inducing apoptosis in myeloma cells. - **Lenalidomide**: An immunomodulatory agent with antiangiogenic and antineoplastic properties, enhancing immune response against myeloma cells. - **Dexamethasone**: A corticosteroid that reduces inflammation and has cytotoxic effects on lymphoid malignancies. - **Cilta-cel (ciltacabtagene autoleucel)**: A BCMA-directed CAR-T cell therapy where patient T cells are engineered to target B-cell maturation antigen (BCMA) on myeloma cells. This combination brings together targeted immunotherapy (daratumumab), proteasome inhibition (bortezomib), immunomodulation (lenalidomide), steroid therapy (dexamethasone), and cellular therapy (Cilta-cel). It is designed for deep disease control in multiple myeloma by attacking malignant plasma cells through complementary mechanisms. While quadruplet regimens like daratumumab plus bortezomib, lenalidomide, and dexamethasone have become new standards of care for newly diagnosed or relapsed/refractory multiple myeloma[1][4][5][7], the addition of Cilta-cel represents an investigational approach aiming for even deeper responses.
