A combination regimen used primarily as first-line therapy for adults with newly diagnosed multiple myeloma who are ineligible for autologous stem cell transplantation. This regimen combines: - **Daratumumab**: a monoclonal antibody targeting CD38 on myeloma cells, mediating cell death via multiple immune mechanisms (complement-dependent cytotoxicity, antibody-dependent cell-mediated cytotoxicity/phagocytosis, apoptosis). - **Bortezomib**: a proteasome inhibitor disrupting protein degradation in myeloma cells, leading to cell cycle arrest and apoptosis. - **Melphalan**: an alkylating agent causing DNA crosslinking and tumor cell death. - **Prednisone**: a synthetic corticosteroid with anti-inflammatory and immunosuppressive properties, inducing apoptosis and augmenting anti-myeloma effects. D-VMP improves progression-free and overall survival versus bortezomib, melphalan, prednisone alone in transplant-ineligible patients with newly diagnosed multiple myeloma[1][3][5][6][7].
