Overview

D-VTd is a quadruplet therapy regimen combining the anti-CD38 monoclonal antibody daratumumab with the proteasome inhibitor bortezomib, the immunomodulatory agent thalidomide, and the corticosteroid dexamethasone. This specific combination is being investigated by The First Affiliated Hospital of Soochow University in a prospective, multicenter observational study focusing on newly diagnosed multiple myeloma (NDMM) patients who also present with renal dysfunction. The regimen leverages multiple mechanisms of action: daratumumab induces immune-mediated tumor cell lysis; bortezomib disrupts protein homeostasis; thalidomide modulates the immune microenvironment and inhibits angiogenesis; and dexamethasone provides synergistic anti-myeloma activity and manages inflammation. The study aims to evaluate the safety and efficacy of this intensive induction and consolidation approach in a real-world clinical setting for a high-risk patient population.
