DCDP is a combination therapy regimen consisting of daratumumab, cyclophosphamide, dexamethasone, and pomalidomide, primarily investigated for the treatment of relapsed and refractory multiple myeloma (RRMM). Daratumumab is a human IgG1k monoclonal antibody that targets the CD38 epitope, inducing tumor cell death through antibody-dependent cellular cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and antibody-dependent cellular phagocytosis (ADCP). Cyclophosphamide is an alkylating agent that cross-links DNA, interfering with DNA replication and cell division. Dexamethasone is a potent synthetic glucocorticoid that exerts anti-inflammatory and immunosuppressive effects while also inducing apoptosis in myeloma cells. Pomalidomide is an immunomodulatory imide drug (IMiD) that binds to the cereblon E3 ubiquitin ligase complex, leading to the degradation of transcription factors Ikaros (IKZF1) and Aiolos (IKZF3), which results in direct antineoplastic activity and T-cell activation. This quadruple combination is being evaluated by the Canadian Myeloma Research Group to improve outcomes in patients who have failed prior lines of therapy.
