DARA-DCEP is a chemoimmunotherapy combination regimen consisting of the CD38-targeting monoclonal antibody daratumumab and the DCEP chemotherapy backbone, which includes dexamethasone, cyclophosphamide, etoposide, and cisplatin. This regimen was specifically evaluated in a Phase II clinical trial (NCT04065308) led by Seoul National University Hospital for the treatment of relapsed or refractory multiple myeloma, particularly in patients presenting with extramedullary disease such as plasmacytomas. Daratumumab induces cell death in CD38-expressing myeloma cells through immune-mediated mechanisms like antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). The DCEP components provide intensive cytotoxic activity: cyclophosphamide and cisplatin act as DNA alkylating and cross-linking agents, etoposide inhibits DNA topoisomerase II, and dexamethasone acts as a glucocorticoid receptor agonist to induce apoptosis and reduce inflammation.
