Daratumumab + hyaluronidase-fihj + dexamethasone is a combination therapeutic regimen primarily used for the treatment of multiple myeloma and light chain (AL) amyloidosis. The regimen is anchored by **Darzalex Faspro**, a subcutaneous formulation containing daratumumab, a human IgG1 monoclonal antibody targeting CD38, and hyaluronidase-fihj, a recombinant human hyaluronidase enzyme that facilitates subcutaneous delivery by temporarily increasing tissue permeability. Daratumumab exerts its anti-tumor effects through complement-dependent cytotoxicity (CDC), antibody-dependent cell-mediated cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), and the induction of apoptosis. Dexamethasone, a potent corticosteroid, acts synergistically with daratumumab to enhance malignant cell death and is also utilized to mitigate potential injection-related reactions. This combination is often further combined with other agents such as lenalidomide, bortezomib, or pomalidomide in various lines of therapy.
