Darolutamide + radiotherapy is a combination therapeutic regimen being investigated for the treatment of oligometastatic castrate-resistant prostate cancer. The regimen consists of darolutamide (Nubeqa), a non-steroidal small molecule androgen receptor (AR) antagonist, and local consolidation radiotherapy, typically delivered as stereotactic ablative body radiotherapy (SABR). Darolutamide functions by competitively inhibiting androgen binding to the AR, inhibiting nuclear translocation of the AR, and blocking AR-mediated DNA transcription. This specific combination is the focus of the Phase 2 DECREASE trial (NCT04319783), sponsored by the Trans Tasman Radiation Oncology Group (TROG). The trial targets patients who are classified as non-metastatic (M0) by conventional imaging but are found to have limited metastatic disease (1-5 metastases, M1) via PSMA PET imaging, aiming to determine if adding metastasis-directed radiation to standard AR-targeted therapy improves progression-free and overall survival.
