**DaRT + mFOLFIRINOX** is a combination therapy integrating diffusing alpha-emitter radiation therapy (**DaRT**) and the modified FOLFIRINOX (**mFOLFIRINOX**) chemotherapy regimen. DaRT is a local radiotherapeutic procedure where seeds loaded with the radioisotope ^224^Ra are implanted directly into the tumor, releasing alpha-emitting decay products (^220^Rn and daughters) that diffuse through and destroy malignant tissue due to their high linear energy transfer and short range. This yields highly localized cytotoxic effects with minimal impact on surrounding healthy tissue, useful for local control of solid tumors, including pancreatic and other resistant cancers[2][4][6]. Modified FOLFIRINOX is an intravenous multi-agent chemotherapy regimen for pancreatic cancer, consisting of reduced-dose irinotecan, omission of 5-FU bolus, and standard doses of oxaliplatin, leucovorin, and 5-fluorouracil, aiming to increase tolerability while retaining efficacy[3][7]. The combination is being explored for synergistic anti-tumor effects, leveraging both local radiotherapy and systemic chemotherapy in difficult-to-treat solid tumors such as locally advanced or recurrent pancreatic adenocarcinoma.
