This combination antiretroviral therapy (ART) regimen was evaluated by Hoffmann-La Roche for the treatment of HIV-1 infection in triple-class treatment-experienced patients. The regimen consists of the protease inhibitor darunavir, the fusion inhibitor enfuvirtide (Fuzeon), and a background of other antiretroviral drugs (typically nucleoside reverse transcriptase inhibitors). Darunavir inhibits the HIV-1 protease enzyme, preventing the cleavage of gag-pol polyproteins and resulting in the production of immature, non-infectious virus particles. Enfuvirtide is a biomimetic peptide that binds to the gp41 subunit of the viral envelope glycoprotein, preventing the conformational changes required for the fusion of the viral and host cell membranes. This multi-targeted approach is designed to overcome resistance in patients who have failed multiple previous therapy classes, such as NRTIs, NNRTIs, and earlier protease inhibitors.
