Darunavir + ritonavir is an antiretroviral combination medication used for the treatment of human immunodeficiency virus type 1 (HIV-1) infection. Darunavir is a protease inhibitor that works by selectively inhibiting the cleavage of HIV-1 encoded Gag-Pol polyproteins in infected cells, which prevents the formation of mature, infectious viral particles. Ritonavir, while also a protease inhibitor, is primarily included in this combination as a pharmacokinetic booster. It achieves this by inhibiting the cytochrome P450 3A4 (CYP3A4) enzyme, which metabolizes darunavir. This inhibition increases darunavir's plasma concentrations and prolongs its half-life, thereby enhancing its antiviral efficacy and allowing for less frequent dosing. The combination is typically administered with other antiretroviral agents to both treatment-naïve and treatment-experienced adult and pediatric patients.
