Overview

Daunorubicin-d4 (Dau-d4) is a small molecule inhibitor that specifically targets microbial-derived dipeptidyl peptidase 4 (DPP4). While host-derived DPP4 is a well-known target in metabolic diseases, recent research has identified a microbial source of the enzyme, produced by gut bacteria such as *E. coli*, which plays a critical role in the progression of intestinal fibrosis in Crohn's disease. Microbial DPP4 acts as a fibroblast activator, triggering the PI3K–AKT signaling pathway and promoting extracellular matrix deposition and stricture formation. Daunorubicin-d4 is being investigated for its ability to selectively inhibit this microbial enzyme, thereby addressing a key unmet need in the treatment of fibrotic complications. Preclinical evidence suggests that combining Dau-d4 with host DPP4 inhibitors like sitagliptin provides a synergistic therapeutic effect, more effectively suppressing collagen deposition and reversing fibroblast activation than single-source inhibition.
