Davoceticept (ALPN-202) is a first-in-class, conditional CD28 costimulator and dual immune checkpoint inhibitor developed for the treatment of cancer. It is an Fc fusion protein composed of a variant immunoglobulin domain of CD80 fused to a human IgG1 Fc fragment. Davoceticept is designed to provide tumor-localized, PD-L1-dependent CD28 co-stimulation while simultaneously inhibiting both the PD-L1 and CTLA-4 immune checkpoints. This tri-functional mechanism aims to enhance antitumor immunity by activating T cells in the tumor microenvironment while blocking inhibitory signals that suppress immune responses. The drug has been evaluated as monotherapy (NEON-1 trial) and in combination with pembrolizumab (NEON-2 trial) in adults with advanced solid tumors or lymphoma[2][3][5][6].
