DB107-FC + DB107-RRV + temozolomide is an investigational combination therapy for high-grade gliomas, including glioblastoma multiforme (GBM) and anaplastic astrocytoma. The regimen consists of three components: - **DB107-RRV** (vocimagene amiretrorepvec): a retroviral replicating vector gene therapy designed to selectively infect tumor cells. Once inside the tumor cell, it delivers a gene encoding cytosine deaminase. - **DB107-FC** (extended-release 5-fluorocytosine): an oral prodrug that crosses the blood-brain barrier and is converted by the enzyme produced from the delivered gene into 5-fluorouracil (5-FU), a potent chemotherapeutic agent, within infected tumor cells. - **Temozolomide**: a standard-of-care oral alkylating chemotherapy used in high-grade glioma treatment. The mechanism aims to achieve targeted chemotherapy production within cancer cells while minimizing systemic toxicity. This approach may also stimulate anti-tumor immune responses. The combination is being evaluated in newly diagnosed and recurrent high-grade gliomas as part of multimodal regimens with surgery and radiation[2][4][7].
