DB818 is a synthetic small molecule inhibitor that targets the transcription factor Homeobox A9 (HOXA9). HOXA9 plays a critical role in regulating hematopoiesis and leukemia cell proliferation and is implicated as a driver of acute myeloid leukemia (AML), particularly in cases with HOXA9 overexpression. DB818 acts by binding to the minor groove of DNA at the HOXA9 cognate sequence, thereby inhibiting the interaction between HOXA9 and its target DNA. This inhibition leads to suppression of AML cell growth, induction of apoptosis, downregulation of key oncogenic transcriptional targets such as MYB, MYC, and BCL2, and upregulation of FOS. In vitro studies have shown that DB818 effectively suppresses growth in several AML cell lines with high susceptibility observed in OCI/AML3, MV4-11, and THP-1 cells. The compound also induces differentiation markers such as CD11b in certain contexts. While primarily developed for research use targeting AML associated with HOXA9 overexpression, some off-target effects have been noted[2][3][6][9].
