Overview

DD-4A is a multi-agent combination chemotherapy regimen primarily utilized in the treatment of pediatric Wilms tumor (nephroblastoma), specifically for patients with Stage III or IV disease or those with focal anaplasia. The regimen consists of three cytotoxic agents: vincristine, dactinomycin (actinomycin D), and doxorubicin (adriamycin). Vincristine acts as a microtubule inhibitor by binding to tubulin and disrupting mitotic spindle formation. Dactinomycin intercalates into DNA and inhibits RNA synthesis by blocking RNA polymerase. Doxorubicin serves as a topoisomerase II inhibitor and DNA intercalator, inducing double-strand breaks and preventing DNA repair. This protocol was developed through the National Wilms Tumor Study (NWTS) and is a standard-of-care backbone used by the Children's Oncology Group (COG). In clinical research conducted at St. Jude Children's Research Hospital, the DD-4A regimen is frequently evaluated in conjunction with advanced radiation techniques, such as proton beam radiation therapy (PBRT), to improve local control while mitigating long-term musculoskeletal and organ-specific toxicities in pediatric survivors.
