Overview

**Decitabine + cedazuridine + venetoclax** is an **all-oral combination therapy** being developed for adults with newly diagnosed acute myeloid leukemia (AML) who are ineligible for intensive induction chemotherapy. Decitabine is a nucleoside metabolic inhibitor (hypomethylating agent) that incorporates into DNA and inhibits DNA methyltransferase, resulting in DNA hypomethylation and cellular differentiation/apoptosis. Cedazuridine is a cytidine deaminase inhibitor that prevents the degradation of decitabine in the gut and liver, enabling effective oral administration. Venetoclax is a small molecule, selective BCL-2 inhibitor that promotes apoptosis in cancer cells by disrupting the anti-apoptotic effects of BCL-2. The combination leverages the synergistic effects of hypomethylating agents and BCL-2 inhibition. The regimen has demonstrated efficacy and safety in phase 2 trials, with a complete response rate of approximately 46-57% in AML patients unable to tolerate intensive chemotherapy. Decitabine/cedazuridine is currently approved as a fixed-dose oral formulation (brand name Inqovi) for myelodysplastic syndrome (MDS), while venetoclax (brand name Venclexta) is approved for various hematologic malignancies. The new combination is under regulatory review for AML and is being explored for post-transplant maintenance in AML/MDS[1][3][5][7].
