Overview

A combination chemotherapy and conditioning regimen comprising decitabine, fludarabine, and busulfan. Developed by Hackensack Meridian Health, this regimen is utilized as a reduced-intensity conditioning (RIC) protocol for patients with myeloid malignancies, including acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS), who are candidates for allogeneic hematopoietic stem cell transplantation. Decitabine serves as a hypomethylating agent that may enhance the sensitivity of malignant cells to subsequent chemotherapy by modulating the epigenetic state of the leukemia cells. Fludarabine, a purine analog, provides potent immunosuppression to prevent graft rejection, while busulfan, an alkylating agent, provides myeloablation to clear the bone marrow for donor cells.
