deferiprone + deferoxamine is a combination iron chelation therapy regimen consisting of the oral bidentate chelator deferiprone and the parenteral hexadentate chelator deferoxamine. This specific sequential alternating protocol was evaluated in a Phase IV clinical trial by Azienda Ospedaliera V. Cervello for the management of iron overload in patients with thalassemia major. The regimen typically involves four days of deferiprone followed by three days of deferoxamine. This approach leverages the 'shuttle effect,' where the smaller deferiprone molecule accesses intracellular iron and transfers it to the more stable deferoxamine for excretion, thereby enhancing iron clearance and potentially reducing the toxicity associated with high-dose monotherapy.
