Defoslimod (OM-174) is a synthetic, triacylated derivative of lipid A that functions as a Toll-like receptor 4 (TLR4) agonist. Developed as an immunomodulatory agent, it is designed to trigger the innate immune system's anti-tumor response by mimicking the immunostimulatory properties of lipopolysaccharide (LPS) with a significantly improved safety profile. The drug promotes the release of pro-inflammatory cytokines and the activation of effector cells such as Natural Killer (NK) cells and macrophages. Clinical investigation, notably led by the Centre Hospitalier Universitaire Dijon, has focused on its safety and efficacy in patients with refractory solid tumors, evaluating its ability to overcome immune tolerance in the tumor microenvironment.
