Delamanid is a small molecule antibiotic from the nitro-dihydro-imidazooxazole class, used primarily for the treatment of multidrug-resistant and extensively drug-resistant tuberculosis (MDR-TB and XDR-TB). It acts as a prodrug that is activated within mycobacteria by the deazaflavin-dependent nitroreductase enzyme (Ddn), leading to the production of reactive metabolites. These metabolites inhibit key enzymes involved in mycolic acid synthesis—specifically methoxy-mycolic and keto-mycolic acids—which are essential components of the mycobacterial cell wall. This disruption impairs cell wall formation, resulting in bactericidal activity against *Mycobacterium tuberculosis*. Delamanid does not display cross-resistance with other anti-tuberculosis drugs and has no significant activity against non-mycobacterial species. The drug was developed to address challenges associated with resistance to standard TB therapies such as isoniazid and rifampin, offering improved outcomes when used in combination regimens for resistant TB cases[1][2][3][4][6].
