Delandistrogene moxeparvovec + imlifidase is a combination therapy under clinical investigation for Duchenne muscular dystrophy (DMD) in patients with pre-existing antibodies to adeno-associated virus (AAVrh74). Delandistrogene moxeparvovec is an adeno-associated virus rh74 (AAVrh74)-based gene therapy that delivers a functional micro-dystrophin gene to muscle cells, aiming to restore dystrophin protein expression and improve muscle function. Imlifidase is a recombinant cysteine protease enzyme that inactivates IgG antibodies by cleaving them, allowing patients with pre-existing anti-AAVrh74 antibodies to receive gene therapy. This combination is administered via intravenous infusion. The approach is studied primarily in DMD patients who would otherwise be ineligible for gene therapy due to antibody-mediated exclusion.[1][9][5][7]
