Overview

Delayed cold-stored buffy coat platelets are a blood-derived therapeutic product consisting of platelets isolated from the buffy coat layer of whole blood and subsequently stored at refrigerated temperatures (1-6°C) after an initial period of room temperature storage. Conventional platelet storage at room temperature (20-24°C) is limited to 5-7 days due to the risk of bacterial contamination and the development of a "platelet storage lesion," which degrades platelet function. Cold storage is being investigated as a method to extend shelf life and enhance the immediate hemostatic efficacy of platelets, as refrigerated platelets exhibit increased procoagulant activity and better adhesion at sites of vascular injury. The "delayed" aspect of this formulation refers to the initiation of cold storage after a specific interval (typically 3 to 5 days) of standard room-temperature holding, which may optimize both availability and hemostatic performance in acute bleeding settings such as cardiac surgery.
