Delipidated high-density lipoprotein (delipidated HDL) is an autologous biologic therapy under investigation for the treatment of acute coronary syndrome (ACS) and atherosclerosis. The therapy involves the selective delipidation of a patient's own plasma HDL using a specialized system, such as the Plasma Delipidation System (PDS-2), which converts large, lipid-rich HDL particles into small, lipid-poor, "pre-beta-like" HDL particles. These nascent particles are highly enriched with apolipoprotein A-I (ApoA-I) and serve as potent acceptors for cellular cholesterol. The primary mechanism of action is the stimulation of cholesterol efflux from lipid-laden macrophages within atherosclerotic plaques, primarily through the ATP-binding cassette transporter A1 (ABCA1) pathway. By facilitating reverse cholesterol transport, the therapy aims to reduce coronary atheroma volume and stabilize vulnerable plaques. While primarily studied in ACS, the therapy has also been utilized in patients with homozygous familial hypercholesterolemia (HoFH).
