Delocamten (MYK-224) is a next-generation, selective cardiac myosin inhibitor developed by Bristol Myers Squibb following its acquisition of MyoKardia. It is designed to address the underlying pathophysiology of heart failure with preserved ejection fraction (HFpEF) and hypertrophic cardiomyopathy (HCM) by reducing cardiac hypercontractility and improving diastolic function. Delocamten works by binding allosterically to cardiac myosin, shifting the myosin population toward a relaxed, "super-relaxed" (SRX) state. This mechanism decreases the number of active actin-myosin cross-bridges during systole, thereby reducing excessive contractility and facilitating better ventricular filling during diastole. It is currently being evaluated in Phase 2 clinical trials for HFpEF.
