Overview

Delolimogene mupadenorepvec is an immunostimulatory oncolytic adenovirus, also known as LOAd 703, developed for cancer therapy. It is a conditionally replicating adenovirus serotype 5/35 genetically engineered to express two key immune-stimulating proteins: a trimerized membrane-bound CD40 ligand (TMZ-CD40L) and the 4-1BB ligand (4-1BBL). The virus selectively replicates in tumor cells with dysregulated retinoblastoma pathways and has deletions in viral genes to enhance immune-mediated killing. Its mechanism of action involves direct oncolysis of tumor cells and potent activation of the immune system by stimulating dendritic cells, macrophages, T cells, and natural killer (NK) cells. CD40 ligand (CD40L) expression enhances antigen presentation and T cell priming via dendritic cell maturation, while 4-1BB ligand (4-1BBL) promotes proliferation and activation of T cells and NK cells. This dual stimulation disrupts the immunosuppressive tumor microenvironment, increases tumor immunogenicity, induces apoptosis in CD40-positive tumors, reduces myeloid suppressor cell populations within tumors, and augments responses to checkpoint inhibitors such as anti-PD-L1 therapies[3][5][6]. Delolimogene mupadenorepvec is being developed primarily for pancreatic cancer (with orphan drug status), malignant melanoma (Phase I/II), breast cancer (research phase), sarcoma (research phase), B-cell lymphoma (no current development), multiple myeloma (no current development)[3].
