DeltEx + temozolomide is a combination therapy for glioblastoma multiforme (GBM) that consists of autologous or allogeneic ex vivo expanded, activated, gene-modified γδ T cells (DeltEx drug-resistant immunotherapy, DeltEx DRI) administered in combination with temozolomide chemotherapy. The γδ T cells are genetically engineered to express O6-methylguanine-DNA methyltransferase (MGMT), which confers resistance to temozolomide-induced cytotoxicity, allowing the cells to persist in the presence of alkylating chemotherapy. This approach seeks to synergize the cytotoxic antitumor effects of temozolomide with the targeted immunotherapeutic activity of γδ T cells, which recognize and kill tumor cells via NKG2D ligand-dependent mechanisms. The DeltEx DRI cells are typically delivered intratumorally following tumor resection, with temozolomide administered systemically according to the Stupp regimen. The therapy is in phase 1b and 2 trials and is being developed for newly diagnosed and recurrent glioblastoma, with ongoing studies evaluating both autologous and allogeneic DeltEx DRI cell products[2][3][4].
