Overview

Demecarium is a long-acting cholinesterase inhibitor and indirect-acting parasympathomimetic agent primarily used in the treatment of glaucoma. It works by inhibiting both acetylcholinesterase and pseudocholinesterase enzymes, thereby prolonging the action of acetylcholine at parasympathetic neuroeffector junctions. This results in constriction of the iris sphincter muscle (miosis) and ciliary muscle, which facilitates increased outflow of aqueous humor from the eye and reduces intraocular pressure. Demecarium is typically reserved for cases where other antiglaucoma agents are insufficient due to its toxicity profile. The drug has also been used in veterinary medicine to manage glaucoma in dogs[1][2][3][5][6][7].
