Dendritic cells pulsed with cytomegalovirus pp65 RNA is an investigational autologous dendritic cell-based cancer vaccine developed primarily at **Duke University** and the **University of Florida**. The therapy involves harvesting a patient's own dendritic cells and transfecting them (often via electroporation) with messenger RNA encoding the **cytomegalovirus (CMV) phosphoprotein 65 (pp65)**. Because CMV antigens like pp65 are expressed in the vast majority of glioblastoma multiforme (GBM) tissues but not in healthy brain cells, the vaccine acts as a targeted immunotherapy. Once re-administered to the patient—typically intradermally—the dendritic cells present the pp65 antigen to the immune system, triggering the expansion of pp65-specific cytotoxic T lymphocytes (CTLs) that migrate to the brain to identify and destroy tumor cells. Clinical protocols often include preconditioning the injection site with **tetanus-diphtheria (Td) toxoid** or using **GM-CSF** to enhance dendritic cell migration to lymph nodes and improve overall efficacy.
