*Dendrobium huoshanense* is a rare medicinal orchid endemic to Huoshan County, China, widely utilized in Traditional Chinese Medicine (TCM) for its immunomodulatory, anti-inflammatory, and gastroprotective properties. Its primary bioactive constituents are polysaccharides (DHPs), alongside flavonoids and alkaloids. In clinical settings, it is being investigated as an adjuvant therapy in neoadjuvant chemoradiation for locally advanced rectal cancer to enhance immune function and alleviate treatment-induced symptoms like radiation proctitis. Preclinical studies suggest broader therapeutic potential in metabolic disorders (hyperuricemia, type 1 diabetes), neurodegenerative diseases (Alzheimer's and Parkinson's), and inflammatory bowel diseases (ulcerative colitis) through mechanisms involving the regulation of the HPA axis, antioxidant pathways (Nrf2/HO-1), and the gut-microbiota-GLP-1 axis.
