Dengtaiye total alkaloid is a purified botanical drug candidate consisting of the total alkaloid fraction extracted from the leaves of *Alstonia scholaris* (L.) R. Br. Developed by the Kunming Institute of Botany (KIB) of the Chinese Academy of Sciences, this therapeutic agent is primarily intended for the treatment of respiratory conditions such as acute tracheobronchitis, post-infectious cough, and bronchial asthma. The extract is rich in indole alkaloids, including scholaricine, vallesamine, and picrinine, which contribute to its multi-target pharmacological profile. It demonstrates potent antitussive, expectorant, and anti-inflammatory effects. Its mechanism of action involves the relaxation of bronchial smooth muscle—potentially through the modulation of L-type calcium channels and muscarinic receptors—and the activation of mu-type opioid receptors to suppress the cough reflex. The drug has progressed through Phase II and Phase III clinical trials in China as a modernized traditional Chinese medicine (TCM) derivative.
