Denileukin diftitox + pegylated interferon alfa-2a is an investigational combination therapy studied primarily in the context of cancer immunotherapy, including ovarian cancer and cutaneous T-cell lymphoma. Denileukin diftitox is a recombinant fusion protein composed of interleukin-2 (IL-2) fused to diphtheria toxin, which selectively targets and kills cells expressing the high-affinity IL-2 receptor (CD25), such as malignant T cells and regulatory T cells (Tregs). Pegylated interferon alfa-2a is a modified form of interferon alpha with polyethylene glycol attached to increase its half-life; it acts as an immunomodulatory cytokine that enhances antitumor immune responses. The combination aims to deplete immunosuppressive Tregs via denileukin diftitox while boosting tumor-specific immunity through pegylated interferon alfa-2a, potentially overcoming resistance seen with monotherapy. Clinical studies have shown that this combination can produce immunologic and clinical benefits in some patients who did not respond to denileukin diftitox alone[1].
