Overview

Denosumab + radium-223 dichloride is a combination therapy that targets bone metastases through complementary mechanisms. Radium-223 dichloride (Ra-223) is an alpha-emitting bone-seeking radiopharmaceutical that selectively targets areas of increased bone turnover in bone metastases, delivering cytotoxic radiation that causes double-stranded DNA breaks in tumor cells[1][3]. Denosumab is a RANKL inhibitor that prevents bone resorption and skeletal-related events in patients with bone metastases[4][5]. This combination has been studied in various cancers with bone metastases, including castration-resistant prostate cancer (CRPC) and hormone receptor-positive breast cancer[3][6]. Clinical data suggests that the addition of denosumab to radium-223 therapy may provide improved outcomes, including potentially longer overall survival compared to radium-223 alone[5][6].
