Depamal Plus is a fixed-dose combination (FDC) small molecule therapy developed and marketed by Provident Pharmaceuticals for the management of type 2 diabetes mellitus. It consists of dapagliflozin, a selective sodium-glucose cotransporter 2 (SGLT2) inhibitor, and metformin, a biguanide. Dapagliflozin works by inhibiting SGLT2 in the proximal renal tubules, thereby reducing renal glucose reabsorption and increasing urinary glucose excretion. Metformin exerts its effects by decreasing hepatic glucose production, decreasing intestinal absorption of glucose, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization. Metformin's molecular mechanism involves the activation of the AMP-activated protein kinase (AMPK) pathway and the inhibition of mitochondrial complex I in the respiratory chain. Depamal Plus is intended to provide glycemic control through these complementary pathways and is listed as an upcoming product in the Greek pharmaceutical market.
