Deraphan (EXV-802) is a small molecule combination therapy developed by Exciva GmbH for the treatment of agitation associated with Alzheimer's disease dementia. It consists of dextromethorphan, an NMDA receptor antagonist, and EXV-801, a novel triple-acting agent that functions as a CYP2D6 inhibitor and a 5-HT2A/2C receptor inverse agonist. The inclusion of EXV-801 serves a dual purpose: it acts as a pharmacokinetic enhancer by inhibiting the metabolism of dextromethorphan to increase its systemic exposure, and it provides additional therapeutic activity through serotonin receptor modulation. This synergistic mechanism is designed to address a broad range of neuropsychiatric symptoms, including hallucinations, delusions, aggression, and agitation. The product has received Fast Track designation from the US FDA and is currently being evaluated in the Phase 2/3 SERENADA clinical trial.
