Dermatophagoides farinae depigmented and polymerized extract is a modified allergen immunotherapy (AIT) product developed by LETI Pharma for the treatment of house dust mite-induced allergic rhinoconjunctivitis and controlled asthma. The manufacturing process involves depigmentation to remove non-allergenic proteins and pigments, followed by polymerization with glutaraldehyde to create allergoids. This modification significantly reduces the extract's IgE-binding capacity, thereby improving safety by minimizing the risk of systemic allergic reactions during administration. Despite reduced allergenicity, the extract retains its immunogenicity, promoting a shift from a Th2-biased immune response to a Th1 and regulatory T-cell (Treg) response. It also induces the production of allergen-specific IgG4 antibodies that act as blocking antibodies to prevent IgE-mediated mast cell degranulation. The extract is typically provided as a suspension adsorbed on aluminum hydroxide for subcutaneous administration.
